Form Submission Guidelines
1. Complete All Required Fields
Ensure all mandatory fields are filled out accurately. Incomplete forms may delay processing.
2. Include Patient Information
Patient name, tooth numbers, and any relevant medical history that may affect the restoration.
3. Specify Materials and Shade
Clearly indicate your preferred material (Zirconia, e.max, etc.) and shade selection. Include photos if possible.
4. Digital Submission
When submitting digitally through your scanner portal, attach the completed PDF form or include the information in the notes field.
